Application process and initial order
Step 1 -Register for the Stoma Appliance Scheme
Once you have had ostomy surgery, your Stomal Therapy Nurse will assist you to register as a participant in the Stoma Appliance Scheme (SAS), subject to you meeting the eligibility criteria). Your Stomal Therapy Nurse will ask you to complete Part 1 of a Stoma Appliance Scheme Application. Your Stomal Therapy Nurse will then complete part 2 to confirm your eligibility.
Step 2 – Apply for membership of QLD Stoma Association Ltd
To obtain products through the SAS, a SAS participant must be a member of an approved stoma association such as QLD Stoma Association Ltd. Your Stomal Therapy Nurse will usually assist you with becoming a member of QLD Stoma Association Ltd. Otherwise, download our application form or contact the association to request a hard copy of our Application Form.
Step 3 – Submit documentation and your first order to QLD Stoma Association Ltd, and pay the SAS Access Fee
Your Stomal Therapy Nurse will assist you to select ostomy products from the list of products available on the SAS Schedule. The products will be those your Stomal Therapy Nurse has determined are necessary for you to manage your ostomy. This list of products will be become your initial order.
Submit your:
- Stoma Appliance Scheme Application (Parts 1 and 2 to be completed)
- QLD Stoma Association Membership Application,
- Medicare card (or passport for citizens of NZ or Republic of Ireland registering under reciprocal health arrangements)
- Commonwealth concession card (if applicable),
- Initial order
to QLD Stoma Association Ltd by:
- email (scan and attach) to [email protected]
- post to QLD Stoma Association Ltd, PO Box 370, CHERMSIDE SOUTH QLD 4032
- deliver in person to Unit 1/10 Valente Close CHERMSIDE QLD 4032 during our business hours
Please make sure that you include payment for the SAS Access Fee and postage (if applicable) to ensure that your application is processed as quickly as possible.
Once you have been confirmed as a member of QLD Stoma Association Ltd and accepted as a SAS Participant, you can register for our members online ordering portal to place future orders, update your profile information, and check ordering history.
